This Privacy Policy is meant to help you understand what personal information we collect, why we collect it, and how we treat it. This policy also explains how you can exercise certain rights you may have, including how you can access and delete your information. This policy reflects our practices over the past twelve months.

We are a healthcare provider that is required to comply with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). This Privacy Policy does not address our privacy practices concerning any Protected Health Information (“PHI”) we collect as a covered entity under HIPAA, including any PHI we collect via our telehealth services or through our website (such as via our patient portal). California Civil Code § 1798.83 permits California residents to request and obtain from us, once per year and free of charge, information about categories of personal information (if any) we disclosed to third parties for their direct marketing purposes and the name and addresses of all third parties with which we shared personal information in the immediately preceding calendar year. If you are a California resident and would like to make such a request, please submit your request by writing to us using the contact information provided below. Please be aware, however, that we do not currently disclose your personal information to any third parties for their direct marketing purposes.

This Services are not directed to children and children are not eligible to use our Services. We will not knowingly collect information from users under the age of eighteen (18). If we learn that we have collected personal information from a child under 18, we will take prompt steps to delete that information as soon as possible.
